Focus on outcomes, not task theatre

1. Orientation

Time: 5 minutes

Start with the quarter's final OKRs on one page or one board. Show the Objective, final KR scores, and status. That's it. No story-telling yet.

The job here is simple. Get everyone looking at the same facts. If the team needs ten minutes of explanation to understand what happened, the preparation was poor.

Good output: shared factual orientation.

2. Scoring review

Time: 15 minutes

Go OKR by OKR. Each owner gets a short slot to share the final score and a one-sentence driver. Keep it crisp. No open discussion yet.

A useful format is:

  • Final score
  • One sentence on what drove the outcome
  • One sentence on what became harder than expected

4. Carryover decisions

Time: 10 minutes

Weak organisations allow the next cycle to be undermined through inaction. They let incomplete OKRs drift forward by inertia.

Don't do that.

For every incomplete OKR or KR, decide one of three things:

  • Carry forward because it remains strategically relevant and still has a valid design
  • Kill it because the business context changed or it was the wrong target
  • Transform it because the intent still matters but the wording, ownership, or measurement was wrong

5. Operating model changes

This is the part most retrospectives neglect, and it's the part that matters most.

Ask: what changes in how we work next quarter?

Not what do we hope will improve. What will change in the operating rhythm.

Examples of useful outputs:

  • Weekly review shifts from status updates to blocker and dependency escalation
  • One cross-functional KR gets a single accountable owner
  • Planning sessions include a dependency map before final sign-off
  • Leadership agrees rules for changing priorities mid-cycle
  • Teams use Start-Stop-Continue or 4Ls to capture process lessons in a shared format

Examples of bad outputs:

  • “Be more proactive”
  • “Collaborate earlier”
  • “Improve communication”
  • “Be clearer on priorities”

Those aren't decisions. They're wishes.

6. Close and commit

Time: 5 minutes

End with three things only:

  • What we're proud of
  • What we're changing
  • What we're carrying forward

Then assign owners. Put dates against follow-up actions. Store the outputs somewhere visible, not buried in meeting notes.

A retrospective is only finished when the next quarter's system has changed because of it.

The Questions That Expose What Really Happened

A strong agenda keeps the room focused. Strong questions make the room honest.

  • Which assumption in planning turned out to be false?

  • If we ran the same quarter again with different OKRs, what would we change in planning?
    This moves the team away from debating content and towards improving process.

  • Where did strategy and day-to-day work drift apart?

Execution and dependency questions

These questions usually produce the most practical learning.

  • Which OKR miss was visible early but not escalated? Why?
  • Which cross-team dependency created the most drag? Was it named during planning?
  • Where did ownership look clear on paper but fail in practice?
  • What did we review every week that didn't help delivery, and what did we fail to review that mattered?

Listen for operating truths

The first answer is often descriptive. The second answer is often political. The third answer is usually useful.

When someone says, “we had capacity issues,” keep going. Ask what created the constraint, who saw it first, and what decision should have happened earlier. That's where the retrospective stops being performative and starts driving progress.

Turning Insights into Action and Accountability

A retrospective without follow-through is just an expensive conversation.
